Affidavit Support: Form, Section, Sample
Affidavit of support is a form and the purpose of this form is to show that the visa applicants have required sponsorship and they will not become public charges while they are in the United States. However, it is required to file separate affidavit for each applicant. You must sign Form 1-134 with your full name and one should remember that Form 1-135 is under US law under penalty of perjury. The place where you will submit the form depends on many factors like whether the one you are sponsoring is in the United States or outside the United States. The filing is free and there are special instructions for forms also.
Form 1-134
This form starts with Department of Homeland Security, US citizenship and immigration services at the top left and form no, expiry date and Form 1-134, Affidavit of Support on the top right. The information on the form should be typed or printed in black ink. In number 1 there are the name, street number, city, state, zip code if in US and the country name with birth date, birth place and country. Then there are questions about citizenships like through naturalization, parents, marriage and whether the person is lawful citizen or lawful admitted non-immigrant. In number 2 there is age and the date since the person was residing in the United States. In number 3 there are name, gender, age, country, marital status, relationship to sponsor, present address, spouse name, spouse age, child’s age and child’s gender. Number 4 has the purpose, number 5 has the bond and number 6 has mention of different acts. Number 7 has the professional details with property details, in number 8 has dependant details, number 9 has mention of previous affidavit, if any, number 10 has mention about visa petition and number 11 has mention of contributions , if any. Then there is oath or affirmation of the sponsor and signature with date.
Student visa
Student visa are given to the students who have completed application form, given the fee, have valid passport, acceptance letter and one should keep the copy of reference. Students should have the acceptance letter from their institutes, proper documents of grants and scholarship, originals of mark sheets and certificates, all needed financial documents, copy of standardized test scores of TOEFL, CAT, etc, credit transfer document and applying should be done as soon as it can be. The reason behind this is that processing of visa take a long time.
Arrest warrant
This form has the heading which says affidavit in support of criminal complaint, arrest warrant and search warrants. Number 1 has the information about the officer, his or her job profile, past records, experience and work technique. Number 2 has the charges of the person, complaint against his or her, the act number under which the crime is charged, etc. Then it has sections and section 1 has the address and address proof, section 2 has crime details, number 3 has officer’s address information and section 4 has other box numbers. Number 3 has words which say about the source of information and that they are correct. Then there are background details and attachments.
